The Application contains a checklist you must follow in order to be in compliance with the Forsyth County Health Board.
A COPY OF THE COMPLETED APPLICATION MUST BE SENT TO THE FORSYTH COUNTY HEALTH BOARD (Address is in the application packet) AND TO
WADE SCHAIBLE BY 10/23/06.
